How can I request a copy of my medical record?

You can request a copy of your medical record in person—just ask the unit secretary or a Wellfound nurse for the Release of Information Authorization form, or call the Health Information Management (HIM) department at 253-301-5447 to request the form.

Once complete, submit the form to the HIM department. You can mail or fax the signed form to:

What about substance use disorder records?

Substance use disorder records have their own federal governing regulations. You may request release of these records. Information is included on the Wellfound Release of Information Authorization form.

How can I send my medical record to another doctor?

You may also request in writing to have us send a copy of your medical record to your primary care or other provider if you supply the name and fax number. We can also send it electronically through Epic, our health information management system.

Can I request an amendment or correction to my medical record?

Yes, you can request an amendment or correction to your medical record. Ask a Wellfound nurse or contact the HIM department at 253-301-5447 to request the form.

The HIM department will have your medical provider review your request. The provider must approve any requested changes to your medical record.

How long does it take to receive a copy of my medical record?

Please allow 15 days to receive your medical record.

Is there a cost involved?

There is no charge for requests to send medical records to other healthcare facilities for continuity of care. For all other requests, please know there may be a fee associated, depending on the volume of records to copy. There is no fee for requesting an amendment to your medical record.

 

Are there times personal health information can be disclosed without my permission?

Personal health information may be disclosed without patient authorization only for the following reasons:

  • Medical emergency
  • Audit programs (state or federal)
  • Research (by government request)
  • Court order

Note for Providers:

Other hospitals requesting medical records of patients in treatment at Wellfound will first need to obtain the patient’s authorization and signature. If the patient is not currently a patient at Wellfound, it is the provider’s responsibility to obtain the patient’s signature before contacting Wellfound Behavioral Health Hospital with this request.
