What happens after I leave Wellfound?

We know wellness begins to happen when people come home to their communities.  That process begins the moment you turn to us for help.  Throughout your time with us, we work together to build the tools you need to move to a less-intensive care setting and to be more successful at home.

We help you identify symptoms of your most difficult times; to lessen their intensity and help you better manage details of daily life. When you complete your program at Wellfound, you leave with a strong transitional plan with next steps firmly in place. We will help arrange follow-up appointments and connect you to appropriate community resources. Our goal is that you step down from hospitalization with a huge amount of support. We want you to continue to receive good, steady care to continue your growth and healing.
